The Margam tragedy – wider learning for the rail industry

  • This session will reflect on the investigation into the death of two track workers at Margam in south Wales, and explore the wider applicability of the safety learning. 
  • It will focus on the organisational factors that were identified in the RAIB’s investigation, such as change management, corporate culture and management assurance. 
  • It will challenge organisations that were beyond the scope of RAIB’s recommendations to reflect on the learning from this tragic accident when reviewing the operation of their own safety management systems.
